Position: Prevention Social Worker
Status: Full-time Term Position, 37.5 hours per week
Schedule: Core hours: 8:00am-4:00pm Flexible (including evenings and weekends) Reporting to: Director of Health
Salary: $73,125 – $87,750 annually
Summary of Position:
Responsible for helping individuals and families develop skills and acquire resources needed to enhance and improve social functioning. Provides appropriate referrals, and culturally appropriate and holistic social services, programs, and resources. This role requires the Social Worker to assess and interview clients, record accurate and detailed case notes and develop and maintain sustainable relationships with children, families, staff and other professionals.
Essential Duties and Responsibilities:
- Intake cases, following relevant policies and procedures, including crisis management, when necessary,
- Interview client(s), family, or group to assess the situation and determine the types and level of services
- Consult with doctors, therapists, and medical professionals as required,
- Direct individuals/ families to available social services and resources to assist with the resolution of personal, family, and social challenges,
- Provide child welfare advocacy, Act as an advocate for clients and perform court-related services as directed,
- Maintain accurate and detailed case notes and records of each visit, conversation, and any other forms of communication,
- Develop and implement an interdisciplinary case management system,
- Maintain sustainable working relationships with community, staff, agencies, schools, law enforcement/ courts,
- Participate in interdisciplinary case management, in regular meetings and communications with SCFSA/MCFD and to other key staff/organizations, as appropriate,
- Keep Director informed of caseload and conduct appropriate reporting procedures,
- Protocol development with SCFSA/MCFD
- Perform other duties as
Qualifications
- Bachelor of Social Work (BSW) from a recognized post-secondary institution with a valid Social Work Designation, license, or registration, as required by the province.
- Minimum of three years or related experience, including case management, counselling, crisis intervention, reporting,
- Good knowledge of social service systems and community resources,
- Demonstrated client assessment, interviewing, counselling, and mediation skills,
- Understanding of crisis management and the ability to deal with life threatening situations, including prevention of suicide,
- Demonstrated ability to interact with people in a sensitive, tactful, diplomatic, and professional manner,
- Ability to work effectively within a multidisciplinary team, independently with strong interpersonal, conflict resolution skills,
- Advanced communication skills required, both oral and written, good organizational, time management and prioritizing skills,
- Must have a class 5 C Drives license and a reliable vehicle,
- Ability to maintain a high level of confidentiality in all interactions,
- Understanding of First Nations traditional healing methods, participate in First Nations protocols, ceremonies, & practices,
- Willing to participate in job-related workshops or training to further skills, knowledge and competencies as required,
- Must be willing to travel, as required, must be able to work occasional evening and weekend shifts,
- Previous experience working in a First Nations community and/or organization is preferred,
- Current and satisfactory Criminal Record Check (vulnerable sector)
